Experienced in hospice and palliative care nursing, with an emphasis on community health education. The nurse case manager plans, organizes, and directs hospice care under hospice physician orders. The ideal candidate must demonstrate strong organizational, interpersonal and communication skills as a member of the interdisciplinary team to meet both the individual patient and family needs. They should be able to develop and follow a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
Qualifications:
- Current MA nursing license.
- Graduate of National League for Nursing accredited school of nursing
- Hospice and/or Home Health experience preferred
- Two+ years of hospice nursing experience required
- Two years Med-surge experience preferred but not required
- High proficiency with computers: Microsoft Office, cellular technology, and EMR required
- Must maintain a state drivers’ license in good standing, and maintain an automobile in good working order
- Fluent in both English and Portuguese language preferred
- Strong teamwork, organization and communication skills
