Medicaid/Medicare Assessor/Registered Nurse

Seeking an MDS Assessor to be responsible for coordinating assessments and completing MDS forms for all residents in accordance with State and Federal regulations and facility policies. This role involves obtaining necessary information, preparing schedules, inputting data, and ensuring accurate and timely transmission of MDS/PRI files to the Department of Health (DOH). Additionally, the MDS Assessor collaborates with the Quality Improvement Department and performs related duties.

 

Compensation:  $110,000 – 122,000

 

Responsibilities:

 

·         Obtains necessary information for thorough and accurate assessments in the MDS process.

·         Reviews the daily census and prepares interim schedules/significant changes for Medicaid and Medicare MDS assessments daily.

·         Inputs data for Medicaid and Medicare in American Health Care Systems from MDS booklets.

·         Prepares and transmits MDS/PRI files to DOH using IBM Dialer and Netscape timely and accurately.

·         Pulls Quality Indicator Reports from state software as required.

·         Files MDS booklets on residents' charts.

·         Orders supplies for the MDS office.

·         Inputs all data for PRI full house, quarterlies, and discharges.

·         Distributes reports to leadership.

·         Works closely with the Quality Improvement Department.


Qualifications:

 

·         Bachelor's Degree in Nursing.

·         Minimum five (5) years of experience in clinical nursing.

·         Two (2) years of experience in performing MDS completion.

·         Licensed Registered Nurse with NYS.
