Locums Physician Assistant Community Liaison Provider

Non-profit healthcare organization seeks a Community Physician Assistant to collaborate with contracted community primary care physicians (CPCPs) for ensuring proper oversight and coordination of participant care within the Interdisciplinary Team. The role involves coordinating and supporting participants' functional, clinical, and psychosocial needs, prioritizing independence and wellness. Responsibilities include serving as a liaison between participants, community physicians, and plan staff to develop and implement care plans, including person-centered goal development, communicating changes in participant condition, and facilitating primary care preventative services.


This is a full-time position, working 5 days per week.


Compensation: $80.00 per hour.


Requirements:


Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.

Minimum of two years in clinical nursing practice in-home care, care management, or related field required.

Adult and geriatric experience required.

Licensed to practice as a Physician Assistant in NY required.

DEA Certification required.

AANP or ANCC Board Certification required.
