Hospice Registered Nurse Case Manager

Full Time

We are seeking a compassionate, experienced, and dedicated Hospice Registered Nurse Case Manager to join our team in the Framingham area. This Monday through Friday full-time position offers an excellent opportunity to make a meaningful impact on patients and families by providing exceptional end-of-life care in the comfort of patients’ homes or LTC settings. As a key member of our interdisciplinary team, you will work closely with physicians, social workers, chaplains, and other healthcare professionals to ensure the highest quality care for our patients.

Key Responsibilities:

  • Patient Care Management: Oversee and manage a designated caseload of hospice patients, providing nursing assessments, care planning, and direct patient care.
  • Clinical Coordination: Collaborate with physicians, social workers, chaplains, and other care team members to ensure comprehensive care.
  • Symptom Management: Provide symptom control and pain management to enhance the comfort and quality of life for patients.
  • Family Support: Offer education and emotional support to patients’ families, helping them navigate end-of-life decisions and the grieving process.
  • Documentation: Maintain accurate and up-to-date patient records, ensuring compliance with all regulatory requirements and organizational standards.
  • Education & Advocacy: Educate patients, families, and caregivers on hospice services, rights, and available resources. Advocate for the patient’s needs within the interdisciplinary team.
  • On-Call Duties: Participate in an on-call rotation to provide nursing support outside of normal business hours.

Qualifications:

  • Registered Nurse (RN) License: Current and unrestricted RN license in MA.
  • Experience: At least 2 years of clinical nursing experience, with a preference for experience in hospice, palliative care, or end-of-life care.
  • Clinical Knowledge: Strong understanding of symptom management, pain relief, and the physical, emotional, and psychological needs of hospice patients and their families.
  • Compassion & Empathy: A genuine desire to provide compassionate care to patients at the end of life.
  • Team-Oriented: Ability to collaborate with an interdisciplinary team to create and execute comprehensive care plans.
  • Communication Skills: Strong interpersonal and communication skills, with the ability to educate and support patients and their families through difficult decisions.
  • Organizational Skills: Ability to manage a caseload, prioritize tasks, and maintain accurate documentation in a timely manner.
