Locums Nurse Practitioner or Physician Assistant – Home-Based Primary Care

We are seeking a Locums Nurse Practitioner or Physician Assistant to join our geriatric care team in Ridgewood, NY. This is a flexible, part-time or full-time role providing in-home primary care to PACE (Program of All-Inclusive Care for the Elderly) participants. The ideal candidate is experienced in home-based or geriatric care, patient-focused, and comfortable working independently in the field.

Position Highlights:

• Flexible Schedule: 2–4 days per week, Monday–Friday
• No nights or weekends
• Low patient volume: 3–5 patients per day
• Hourly Compensation: $75–$80 per hour

Responsibilities:

• Provide comprehensive primary care services in participants' homes
• Conduct routine and urgent visits, including history, physical exams, medication reconciliation, and care planning
• Collaborate with interdisciplinary PACE care team to ensure coordinated, high-quality care
• Document clinical encounters in a timely and accurate manner
• Support chronic disease management, preventative care, and advance care planning

Requirements:

• Active Nurse Practitioner or Physician Assistant Board Certification
• Medicare & Medicaid Provider ID – Required
• Minimum 2 years of clinical experience preferred, ideally in geriatrics, primary care, or home care settings
• Spanish-speaking candidates strongly preferred, but not required
• Strong interpersonal and communication skills
• Comfortable practicing independently in home-based environments
