We are seeking a Locums Nurse Practitioner or Physician Assistant to join our geriatric care team in Ridgewood, NY. This is a flexible, part-time or full-time role providing in-home primary care to PACE (Program of All-Inclusive Care for the Elderly) participants. The ideal candidate is experienced in home-based or geriatric care, patient-focused, and comfortable working independently in the field.
Position Highlights:
- Flexible Schedule: 2–4 days per week, Monday–Friday
- No nights or weekends
- Low patient volume: 3–5 patients per day
- Hourly Compensation: $75–$80 per hour

Responsibilities:
- Provide comprehensive primary care services in participants' homes
- Conduct routine and urgent visits, including history, physical exams, medication reconciliation, and care planning
- Collaborate with interdisciplinary PACE care team to ensure coordinated, high-quality care
- Document clinical encounters in a timely and accurate manner
- Support chronic disease management, preventative care, and advance care planning
Requirements:
- Active Nurse Practitioner or Physician Assistant Board Certification
- Medicare & Medicaid Provider ID – Required
- Minimum 2 years of clinical experience preferred, ideally in geriatrics, primary care, or home care settings
- Spanish-speaking candidates strongly preferred, but not required
- Strong interpersonal and communication skills
- Comfortable practicing independently in home-based environments
