Social Worker LISW or LCSW Community Care Manager

The Clinical Social Work Supervisor will be responsible for the overall clinical and operating functions of the Social Workers assigned to each market assignment. Responsibilities include providing a consistent, clear onboarding process for all social workers with their specific regional market community teams. The Clinical SW Supervisor will work to ensure market social workers are educated, motivated, and aligned with the WellBe model of care. The Clnical Supervisor will ensure all social workers in market territories are properly engaged and maintain appropriate communication with those necessary.

Our social workers are part of a team that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. Our LCSW delivers primary, urgent and acute care and counseling to a wide variety of patients with social and behavioral problems. The LCSW will be accountable for caring for patients, connecting patients and their families to support services, maintaining accurate and current patient records and scheduling and administering initial, urgent, and follow-up appointments to patients as required. The successful candidate will work as a team with our physicians, advanced practice clinicians, and care team coordinator will assist in delivering quality care to every patient. The Clinical Social Work Supervisor is responsible for directing the work of professional Clinical Social Workers providing care, treatment, and services. The Clinical Supervisor will work within general methods and procedures and exercises considerable judgment to adapt and apply the guidelines to specific situations.  The work requires knowledge of WellBe policies, procedures, and regulations of clinical social work, and supervisory techniques, personnel policies, and procedures

• Coordinates activities with social work schedules, setting priorities, and directing the work of social workers.
• Participates in program and policy development of the WellBe Social Work program.
• Ability to translate the latest evidenced based models into practical counseling and therapy skills.
• Ability to organize work effectively, conceptualize and prioritize objectives and exercise independent judgment based on an understanding of WellBe policies and activities.
• The clinical supervisor will monitor activities at markets to ensure policies and guidelines are developed and observed. 
• The clinical supervisor will be responsible for collecting clinic data and clinical outcome measures and share reports. 
• Manages complex behavioral and psychosocial needs that result in improved clinical and financial outcomes.
• Performs psychosocial assessments and provides therapeutic services to patients and their families
• Work closely with all care team providers on complex patient cases and transitions of care
• Maintains constant communication with patients when addressing their concerns and goals and helping them keep positive attitudes
• Builds rapport with patients and their families and relay all concerns to provider/IDT
• Following a thorough assessment completed by an Advanced Practice Clinician (APC), the WellBe social worker partners
• Partner with community team to refer patients to appropriate no or low-cost community services that support health and independence such as Meals On Wheels (MOW), local Area Agencies on Aging (AAA), Senior & Assistance Programs, and transportation resources.
• Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to their supervisor/team. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.
• Maintains current written progress notes and other documentation on the member Medical Record including signed and dated documentation for all service performed the day the service is provided

Educational Requirements:

• Master’s Degree in Social Work
• LISW/LCSW Licensure required
• Valid active licensure in Clinical Social Work required
• 5+ years clinical social work experience
• 2+ years of experience in managing and providing clinical supervision required, or extensive experience mentoring, guiding, or precepting for 2+ years. 
• 2+ years social work experience, preferably in health care
• 1+ year experience working with a frail/elderly population. 

Required Skills and Abilities:

• Age specific competency in working with the elderly and knowledge of community services for the elderly and their families.
• Ability to make psychosocial assessments and develop and implement viable care plans
• Strong computer skills, including Word, Excel, and Powerpoint
• Strong verbal, written, presentation, and interpersonal communication skills
• Bilingual in English/Spanish preferred.
• Ability to work effectively in a team environment.
• Knowledge of social work principles and practices, including case management and counseling techniques.
• Ability to provide after-hours services as needed
