Nurse Practitioner or Physician Assistant – Primary Care

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our team in Hominy, Oklahoma. This full-time position offers the opportunity to provide comprehensive primary care services in a community health setting. You will work collaboratively with a multidisciplinary team to promote health, prevent illness, and deliver high-quality care to patients across the lifespan. New graduates are welcome to apply.


Key Responsibilities:

  • Provide comprehensive health assessments and primary care services, including screening, diagnosis, treatment, and health education, within the NP/PA scope of practice.
  • Conduct systematic history-taking and physical examinations.
  • Compile and maintain accurate patient medical records and health histories.
  • Order, perform, and interpret diagnostic and laboratory tests; identify deviations from normal results and initiate appropriate follow-up.
  • Develop and implement individualized treatment plans based on clinical findings.
  • Provide patient counseling, education, and follow-up for acute and chronic health conditions.
  • Collaborate with physicians, nurses, and other healthcare professionals in the delivery of coordinated care.

Requirements:

  • Graduation from an accredited Nurse Practitioner or Physician Assistant program.
  • Current licensure as a Nurse Practitioner or Physician Assistant in the State of Oklahoma.
  • National board certification as a Nurse Practitioner or Physician Assistant
  • Current DEA and Oklahoma prescriptive authority.
  • Current BLS/CPR certification
  • Excellent clinical, diagnostic, and interpersonal skills.
  • Strong organizational skills and the ability to work both independently and as part of a team.
  • Experience in family medicine, internal medicine, or community health preferred.
