We are seeking a licensed Occupational Therapist
(OTR/L) for a temporary opportunity within a PACE (Program of All-Inclusive
Care for the Geriatrics) program located in Flushing, New York. This full-time
role supports participants through direct treatment in the clinic, at home, and
in nursing facilities, with a strong focus on rehabilitation, independence, and
interdisciplinary care coordination.
Assignment Details:
- Schedule: Monday–Friday, 9:00 AM – 5:00 PM
- Setting: PACE Center, participant homes,
nursing facilities, and outpatient sites
- Patient Population: Frail/elderly
adults
- Coverage needed till January 2026
Key Responsibilities:
- Evaluate participants and create
individualized occupational therapy treatment plans
- Administer therapeutic services at the
center and in the field (participant homes and nursing homes)
- Collaborate with an interdisciplinary team
to develop and execute care plans
- Document all care accurately in alignment
with state and PACE regulations (evaluations, progress notes, MDS,
discharge summaries)
- Recommend adaptive equipment and
modifications to improve function and safety
- Participate in quality improvement and
provide clinical instruction to COTAs and students
- Ensure compliance with infection control, safety procedures, and ethical standards
Qualifications:
- New York State licensure as a Registered
Occupational Therapist – Required
- Minimum 1 year of OT experience, preferably
with geriatric populations
- Valid driver’s license with ability to
travel locally
- Strong working knowledge of PACE regulations

- Familiarity with Care Compass and other EMRs
– Preferred
- Excellent communication, documentation, and teamwork skills
