Full Time

Full-Time | $22.83/hour
Schedule: Sunday–Thursday or Tuesday–Saturday | 40 hours/week | 9-hour days with 1-hour lunch
Shift Start Times (ET): 10am, 11am, 12pm, or 1pm
Location: Remote

We’re seeking compassionate, resourceful professionals to support members through every step of their healthcare journey. Ideal candidates bring healthcare experience, care coordination know-how, and the drive to help people navigate complex systems with clarity and empathy.

What You’ll Do:

  • Act as the go-to resource for members, answering questions and resolving needs

  • Coordinate care, schedule appointments, and support post-discharge planning

  • Help members understand and use their insurance benefits effectively

  • Document Social Determinants of Health (SDOH) and connect members with local resources

  • Communicate across multiple channels (phone, email, chat)

  • Maintain accurate records using Salesforce

  • Meet KPIs in a fast-paced, metrics-driven environment

Who You Are:

  • Experienced in care coordination, either professionally or personally

  • Background in healthcare (2+ years strongly preferred)

  • Empathetic, member-focused, and an excellent communicator

  • Calm under pressure, adaptable to change, and highly organized

  • Motivated by service and comfortable juggling priorities

  • Familiar with DME, discharge planning, and case management is a plus

  • Bilingual in Spanish is a bonus (no current differential pay)

Requirements:

  • High school diploma

  • Reliable high-speed internet and a quiet, private home workspace

  • Comfortable working remotely and using CRM platforms (Salesforce preferred)

  • Willingness to occasionally work overtime or cover holidays as needed
