Full-Time | $22.83/hour
Schedule: Sunday–Thursday or Tuesday–Saturday | 40 hours/week | 9-hour days with 1-hour lunch
Shift Start Times (ET): 10am, 11am, 12pm, or 1pm

We’re seeking compassionate, resourceful professionals to support members through every step of their healthcare journey. Ideal candidates bring healthcare experience, care coordination know-how, and the drive to help people navigate complex systems with clarity and empathy.
What You’ll Do:

Act as the go-to resource for members, answering questions and resolving needs

Coordinate care, schedule appointments, and support post-discharge planning

Help members understand and use their insurance benefits effectively

Document Social Determinants of Health (SDOH) and connect members with local resources

Communicate across multiple channels (phone, email, chat)

Maintain accurate records using Salesforce

Meet KPIs in a fast-paced, metrics-driven environment
Who You Are:

Experienced in care coordination, either professionally or personally

Background in healthcare (2+ years strongly preferred)

Empathetic, member-focused, and an excellent communicator
-
Calm under pressure, adaptable to change, and highly organized

Motivated by service and comfortable juggling priorities

Familiar with DME, discharge planning, and case management is a plus

Bilingual in Spanish is a bonus (no current differential pay)
Requirements:

High school diploma

Reliable high-speed internet and a quiet, private home workspace

Comfortable working remotely and using CRM platforms (Salesforce preferred)

Willingness to occasionally work overtime or cover holidays as needed
