Psychiatric-Mental Health Nurse Practitioner / Physician Assistant (PACE Program)

Full Time

We are seeking a full-time, board-certified Psychiatric-Mental Health Nurse Practitioner or Physician Assistant to join our interdisciplinary team in Stockton, CA within the PACE (Program of All-Inclusive Care for the Elderly) model. In this role, you will work collaboratively with PACE physicians and the Interdisciplinary Team (IDT) to provide comprehensive psychiatric care, including assessment, counseling, treatment planning, and care coordination for a frail and elderly population. 

 

Key Responsibilities 

  • Conduct psychiatric evaluations (in-person/telehealth) and help create care plans with the IDT 
  • Provide treatment, counseling, and consultation alongside PACE and community-based physicians 
  • Support discharge planning and ongoing care in acute, home, and long-term settings 
  • Oversee psychiatric care in nursing homes with regular visits as needed 
  • Communicate participant updates and collaborate with the IDT to ensure 24/7 care continuity 

 

Requirements: 

  • Master’s degree in Nursing or completion of accredited Physician Assistant program with a mental health emphasis 
  • Certification as a PMHNP or Psychiatric PA 
  • Active and unrestricted state license 
  • State CDS and DEA registration 
  • Current NPI number 
  • Minimum two (2) years of experience working with elderly or frail populations 
  • Reliable transportation for community-based visits
