We are seeking a full-time Primary Care Nurse Practitioner or Physician Assistant to join our team in Elk Grove, CA as part of the PACE (Program of All-Inclusive Care for the Elderly) model. This role offers a meaningful opportunity to deliver comprehensive, community-based care to older adults in both clinical and home settings, with support from an interdisciplinary care team.
Key Responsibilities:
- Provide primary and preventive care to PACE participants in clinic and at home
- Collaborate with PACE physicians and the interdisciplinary team to develop and adjust care plans
- Participate in a rotating, phone-only on-call schedule (a few nights or weekends per month)
- Ensure continuity of care and manage chronic and acute conditions
- Promote health education, disease prevention, and overall wellness
Compensation & Benefits:
- Salary: $145,399 – $174,399 annually + Quarterly Bonus
- Time Off: 17 PTO days, 6 sick days, 12 paid holidays, weekends off
- Work Hours: Monday–Friday, 8 AM–5 PM with home visits included

Requirements:
- Licensed Nurse Practitioner or Physician Assistant in California
- Prescriptive authority and DEA registration
- Eligible to serve Medicare and Medi-Cal participants
- Minimum of 2 years’ experience caring for elderly or medically complex populations
- Strong communication and team collaboration skills
- Compliance with company COVID-19 vaccination policy is required; proof of full vaccination must be provided as a condition of employment
