A premier PACE (Program of All-Inclusive Care for
the Elderly) program in Bronx, NY is seeking a Locums Nurse Practitioner
Supervisor (NPS) to provide clinical oversight, participant care, and staff
leadership on a full-time basis. This locum tenens opportunity offers a
rewarding mix of direct patient care, interdisciplinary collaboration, and
clinical team supervision focused on geriatric adults.
This position will be predominately field work (home visits) with some potential coverage in clinic as needed.

adults with complex needs

Key Responsibilities:
- Serve as the primary clinical liaison
between community primary care physicians (CPCPs), the participant, and
the interdisciplinary team (IDT)
- Perform annual wellness assessments, urgent
care evaluations, and transitional care visits post-hospital/ER discharge
- Supervise and manage Nurse Practitioners and
assist in oversight of Diagnostic Treatment Center (DTC) clinical staff
- Provide leadership coverage in the absence
of the Medical Director
- Review and maintain accurate medical
histories and diagnostic coding for appropriate risk adjustment
- Participate in clinical documentation
improvement (CDI) and coach staff on documentation quality
- Ensure timely access to preventive and acute
care services, including same-day urgent care
- Coordinate medication reconciliation,
follow-up planning, and communication of any change in condition
- Participate in interdisciplinary care planning and represent clinical leadership during care conferences
Qualifications:
- Graduate of an accredited Nurse Practitioner program
- Licensed Nurse Practitioner in New York State – Required
- DEA Certification – Required
- AANP or ANCC Board Certification – Required
- 2+ years of clinical practice in home care, care management, or related fields – Required
- 1+ year experience working with frail or elderly populations – Preferred (training provided if less than 1 year)
- Strong leadership and interpersonal skills
- High-level clinical decision-making and documentation skills
- Proficiency with EMRs and patient care planning
- Strong understanding of geriatric and chronic care management
