We are seeking an experienced and detail-oriented
Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a
remote locums (per diem) role. This position focuses on chart review and care
coordination, helping to ensure patients receive comprehensive, evidence-based,
and well-coordinated care. The role offers flexibility, allowing you to set
your schedule while supporting high-quality patient outcomes.
Responsibilities:
- Review patient medical records and histories
to identify care gaps and opportunities for improved health outcomes.
- Develop, update, and implement
individualized care plans in collaboration with the clinical team.
- Provide medical input on care coordination
activities to ensure comprehensive and evidence-based approaches.
- Identify and recommend necessary
preventative screenings, immunizations, and interventions according to
best practice guidelines.
- Communicate effectively with patients,
caregivers, and interdisciplinary team members regarding care plans and
next steps.
- Accurately document findings and
recommendations within the EMR system.
- Participate in interdisciplinary team
discussions and contribute to ongoing patient care strategies.
Requirements:
- Current and valid Nurse Practitioner (NP) or
Physician Assistant (PA) license.
- Active New York state license
