What you’ll do

• Provide primary care for long-term care residents
  
  

• Round daily in a skilled nursing facility
  
  

• Manage chronic + acute conditions
  
  

• Support “treat-in-place” goals to reduce hospitalizations
  
  

Requirements

• Board-certified PA
  
  

• Active Pennsylvania license
  
  

• Experience with adults/geriatrics, primary care, or post-acute preferred
  
  

• Comfort working independently in a facility setting
