The Regional Nurse delivers home-based clinical care to PACE participants, ensuring safe, high-quality services in accordance with care plans, physician orders, and nursing standards. This role includes assessments, direct patient care, care coordination, education, and active participation as a member of the Interdisciplinary Team.
Locations & Schedule: Bronx,
NY. Mon–Fri, 8:30 AM–5:30 PM (40 hrs/week)
Responsibilities:
- Perform routine home visits, including wound care, medication administration/reconciliation, and health assessments.
- Average 4 patients a day
- Monitor participant conditions, document accurately in the EHR, and communicate updates to physicians and the care team.
- Educate patients and caregivers on disease management, medication adherence, wound care, and safety.
- Collaborate with care coordinators, partner organizations, and internal departments to meet participant needs.
- Ensure adherence to protocols, infection control, and regulatory standards.
- Participate in case conferences and clinical meetings.
- Travel between homes in assigned service area; act within scope of license and program standards.
Requirements:
- Degree from an NLN-accredited nursing school (RN or BSN).
- Active, unrestricted NY RN license.
- 3+ years of clinical nursing experience; 1–2 years in home care preferred.
- Experience with frail or elderly populations preferred (PACE training provided if less than 1 year).
- Strong assessment, communication, and critical thinking skills; ability to manage multiple cases.
Compensation & Benefits:
- Salary: $120,000–$125,000 annually, based on experience.
