A physician-owned multispecialty group in Fall River, MA is seeking a dedicated Part-Time Nurse Practitioner to join our team. We provide compassionate, patient-centered care to homebound patients and to residents in local skilled nursing facilities. This role offers an opportunity to work independently while collaborating closely with physicians, visiting nurse agencies, and facility medical teams to deliver high-quality, coordinated care.
Position Overview: The
Nurse Practitioner will split time between home visits and SNF rounds
(approximately 2 days in each setting). For candidates who prefer home care
exclusively, scheduling flexibility may be available.
- Home Care Duties (Fall River, New Bedford, Westport, MA):
- Conduct comprehensive patient visits (45–60 minutes per visit; target of 8 patients per day).
- Reconcile medications and educate patients and caregivers.
- Diagnose conditions, order appropriate testing, and coordinate with visiting nurse agencies, PCPs, and specialists.
- Support ongoing care by serving as a clinical resource to visiting nurses.
Skilled Nursing Facility
Duties:
- Work alongside other Nurse Practitioners and facility Medical Directors.
- Evaluate and manage patients (target of 2 patients per hour, 16 per day).
- Develop and implement individualized management plans.
- Provide direct oversight of patient care in collaboration with facility staff.
Schedule & Compensation:
- Part-time: 32 hours per week (2 days SNF, 2 days Home Care; home care–only option available).
- Competitive hourly rate: $60–$65 per hour.

Requirements:
- Current Nurse Practitioner license in the Commonwealth of Massachusetts (Rhode Island licensure a plus).
- Valid DEA certificate and Massachusetts Controlled Substances Registration.
- Demonstrated ability to work independently and collaboratively.
- Strong organizational, communication, and problem-solving skills.
- Comfortable working with interdisciplinary teams, including nurses, therapists, pharmacists, administrators, and case managers.
- Fluency in Portuguese preferred but not required.
