We are seeking a compassionate, highly organized Registered Nurse Clinical Care
Guide to join our Advanced Primary Care team in Scottsdale, AZ. In this
patient-centered role, you will provide end-to-end support for specialist and
diagnostic testing navigation, ensuring patients receive seamless, coordinated
care throughout their health journey. As a key clinical partner, you will
collaborate closely with providers, care coordinators, and support staff to
help patients achieve their best possible health outcomes.

Key Responsibilities:
- Serve as the main point of contact for specialist and diagnostic testing navigation, including scheduling and follow-up.
- Support patients through individualized care plans and promote proactive health management.
- Conduct clinical assessments and provide guidance aligned with the Advanced Primary Care model.
- Assist with care transitions, including post-discharge follow-up and coordination with community resources.
- Maintain accurate electronic medical documentation.
- Provide excellent customer service and build strong patient relationships.
- Perform other duties as assigned.
- Work flexible hours, including mornings, evenings, or weekends.
- Travel between care centers as needed; position is 100% on-site.
Requirements:
- Current Arizona Registered Nurse license.
- BLS certification required.
- Experience with Electronic Medical Records (EMR), including Medicare-based systems.
- Strong customer service orientation with experience in patient-facing roles.
- Excellent verbal, written, and electronic communication skills.
- Active AZ Fingerprint Clearance Card required.
