We are seeking a compassionate and
detail-oriented Behavioral Health Case Manager to support individuals receiving
behavioral health services through care coordination, patient education, and
connection to community-based resources. This role works closely with medical
providers, behavioral health clinicians, and external partners to promote
engagement, continuity of care, and recovery.
The Behavioral Health Case Manager
provides direct case management services to patients enrolled in behavioral
health programs. The role focuses on care coordination, patient engagement,
education, documentation, and collaboration with internal and community
partners to support treatment adherence and positive outcomes.
Compensation: $43,855 - $53,722
Responsibilities
- Provide behavioral health case management services to
support patient engagement, treatment adherence, and continuity of care.
- Coordinate referrals and connect patients to
appropriate medical, behavioral health, and community-based services.
- Engage patients using trauma-informed,
strengths-based, and motivational interviewing approaches to support
recovery and self-management.
- Monitor patient participation, follow up on missed
appointments, and address barriers to treatment.
- Collaborate closely with medical providers,
psychiatric providers, counselors, and external partners to support
integrated care.
- Assist, under clinical supervision, with intakes,
assessments, diagnoses, and development and maintenance of treatment and
service plans.
- Monitor patient symptoms and treatment responses and
communicate changes to the clinical care team.
- Support transitions of care, including coordination
with residential treatment facilities and discharge planning.
- Ensure timely, accurate documentation and billing in
the electronic health record in compliance with AHCCCS and organizational
requirements.
Qualifications
- Education & Experience (one of the following):
- High School Diploma/GED with 4 years of behavioral
health experience, OR
- Associate’s degree in a behavioral health field
with 2 years of behavioral health experience, OR
- Bachelor’s degree in a behavioral health field with
1 year of behavioral health experience
- Valid Fingerprint Clearance Card
- Driver’s license and proof of insurance (if mileage
reimbursement is requested)
- Bachelor’s degree in a behavioral health-related
field preferred
- One (1) year of direct behavioral health case
management experience preferred
- Experience providing patient care in a behavioral
health setting preferred
