Seeking a Case Manager to provide
behavioral health case management, patient education, and linkage to community
resources to support treatment, recovery, and ongoing engagement. This role
collaborates closely with patients, medical providers, counselors, and
community organizations to ensure high-quality, coordinated care.
Compensation: $43,855 - $53,722
Responsibilities:
- Provide case management and care coordination for
behavioral health patients, supporting engagement, adherence, and
recovery.
- Facilitate referrals and connect patients to
appropriate medical, behavioral, and community services.
- Conduct patient follow-ups, monitor clinical
symptoms, and address barriers to treatment.
- Assist with intake assessments, treatment and service
plan development, and maintenance under clinical supervision.
- Support transitions of care, including discharge
planning from residential facilities and outpatient follow-up.
- Apply trauma-informed, strengths-based, and
motivational interviewing approaches to engage patients and families.
- Collaborate with medical providers, psychiatric nurse
practitioners, and counselors to ensure coordinated care.
- Ensure timely, accurate documentation and billing in
the electronic health record in compliance with AHCCCS and organizational
standards.
- Participate in staff development, trainings, quality
improvement initiatives, and community engagement activities as assigned.
Qualifications:
- Bachelor’s degree in behavioral health or related
field.
- Two years of direct case management experience in
behavioral health.
- Valid Fingerprint Clearance Card.
- Driver’s license and proof of insurance if mileage
reimbursement is requested.
- Master’s degree in behavioral health or related field
preferred
- Experience providing care for patients with mental
health diagnoses or co-occurring disorders preferred
