Licensed Practical Nurse

We are seeking a Licensed Practical Nurse (LPN) to provide supportive and restorative nursing care in a Long-Term Care (LTC) Nursing Home and bedside setting under the supervision of a Registered Nurse. The LPN will deliver direct patient care, administer medications and treatments, and assist with a variety of nursing tasks to ensure patient comfort, safety, and overall well-being.

This role involves working with residents requiring long-term, rehabilitative, and skilled nursing care, ensuring compliance with care plans and facility standards while promoting a compassionate, patient-centered environment.

• Provide direct patient care including hygiene, nutrition, ambulation, exercise, and elimination needs
• Administer medications and treatments as assigned per hospital policy
• Monitor patient safety and take vital signs
• Perform CPR and assist with medical procedures as needed
• Prepare and transport patients for tests and procedures
• Document patient care and report status to supervising RN

• Graduate of an accredited nursing program
• Licensed and currently registered as a Practical Nurse in New York State
