We are seeking an experienced and compassionate Care Delivery Manager - Registered Nurse to join our interdisciplinary team in Flushing, NY, supporting a high-quality PACE (Program of All-Inclusive Care for the Geriatrics) care model. In this leadership role, you will oversee daily clinical operations, ensure coordination of services, and support the delivery of culturally competent, person-centered care to our diverse geriatric population.
The candidate should have Managed Long Term Care (MLTC) experience and a strong background in community health and home care, with demonstrated management experience.
Compensation: $55 - $64.50 Per Hour
Schedule: Monday-Friday 8:30 am-5:00 pm
Key Responsibilities
- Oversee development and execution of individualized care plans that promote participant safety, satisfaction, and improved health outcomes
- Lead and participate in Interdisciplinary Team (IDT) meetings, including Service Delivery Requests (SDRs), ensuring regulatory compliance and timely documentation
- Provide clinical leadership and represent Community Health Nurses (CHNs) within IDT discussions
- Review and analyze Clinical Delivery reports to drive care coordination and operational decision-making
- Manage and allocate staffing resources to ensure appropriate coverage and seamless service delivery
- Serve as manager-on-call during designated off-hours to support urgent participant or caregiver needs
- Maintain strong communication across interdisciplinary teams and external partners to ensure care aligns with participant goals and preferences
Qualifications
- Managed Long-Term Care (MLTC) experience required
- Strong background in community health and/or home care, with solid management experience preferred
- Active, unrestricted New York State Registered Nurse (RN) license and registration required
- Degree from an NLN-accredited nursing school (RN required; BSN strongly preferred)
- Supervisory experience required
- Minimum of 2+ years of administrative experience in a management role within CHHA, LHCSA, MLTC, acute care, nursing home, or diagnostic and treatment clinic settings preferred
- Experience working with frail or elderly populations preferred (PACE training provided if needed)
- Case Management Certification recommended within 2 years of employment
- Successful completion of NYSED-approved infection control coursework required
- Minimum of 8 continuing education contact hours, including mandatory training requirements
- Bilingual skills preferred
- Strong customer service, interpersonal communication, and cross-disciplinary collaboration skills
- Demonstrated ability to manage complex care environments and operational priorities
