We are seeking a compassionate and experienced Nurse Practitioner to join our team in Mishawaka, Indiana. This full-time role works closely with clinical staff and reports to the Site Medical Director and Site Operations Director, providing high-quality, patient-centered outpatient care within a Patient-Centered Medical Home (PCMH) model.
Responsibilities:
- Provide comprehensive outpatient primary care and manage patient panels to ensure continuity of care
- Develop and implement effective patient care plans, including referrals as appropriate
- Perform periodic health assessments and provide patient education
- Maintain timely, accurate, and compliant clinical documentation
- Complete a minimum of 2,946 outpatient visits annually
- Participate in quality assurance activities, clinical meetings, and performance audits
- Support quality improvement initiatives and clinical protocols
- Participate in community outreach, education, and student precepting as assigned
Requirements:
- Master’s Degree in Nursing from an accredited Nurse Practitioner program
- Active Indiana RN license and Nurse Practitioner certification
- Prescriptive authority with active DEA registration
- Current CPR certification
- 1–3 years of Nurse Practitioner experience or significant RN experience in ER or ICU
