Locums-to-Perm Spanish Nurse Practitioner

We are seeking an experienced Spanish-Speaking Nurse Practitioner for a Locums assignment with potential to convert to Perm at a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care.

Compensation: $75 – $90 per hour

Position Details:

• Schedule: Full Time Hours Guaranteed
• Monday – Friday - 5 Days Per Week
• Patient Load: LOW DAILY CENSUS
• Home Care: 3–5 patients per day
• Clinic: Small census of 7–10 patients per day
• Setting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients

Responsibilities:

• Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients
• Diagnose, treat, prescribe medications, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging, etc.)
• Document accurately in the EHR system
• Provide patient education and promote wellness initiatives
• Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
• Refer patients to specialists and manage follow-ups
• Ensure compliance with state, federal, and professional regulations

Requirements:

• Active NP license in NY
• Minimum 2 years of primary care experience
• National Board Certification
• Spanish Speaking
• Medicaid and Medicare Provider Id required
