We are seeking a
Physical Therapist to join a multidisciplinary PACE (Program of All-Inclusive
Care for the Elderly) team in Brooklyn, NY. This role provides physical therapy
services to frail and elderly patients in both the clinic and community settings,
helping them improve mobility, restore function, relieve pain, and maintain
independence.
Compensation: $50-$60 Per Hour
Position Details
- Schedule: Monday–Friday, 9:00 AM – 5:00 PM
- Hours: 35 hours per week
- Setting: Clinic and community/home visits
- Patient Population: Geriatric
Responsibilities
- Perform physical therapy evaluations,
treatments, reassessments, and discharge planning.
- Develop individualized treatment plans to
improve strength, mobility, balance, and function.
- Collaborate with physicians and an
interdisciplinary care team.
- Educate patients and caregivers on
exercises, mobility, and fall prevention.
- Complete accurate clinical documentation and
participate in care planning meetings.
Qualifications
- Graduate of an accredited Physical Therapy
program.
- Active New York Physical Therapist license.
- One year of physical therapy experience
preferred.
- Geriatric or PACE experience preferred.
- Ability to travel to participant homes and
community sites.
- Bilingual candidates are encouraged to
