Mobile Nurse Practitioner - Adult/Geriatric

We are seeking a compassionate Mobile Adult/Geriatric Nurse Practitioner to provide acute, post-acute, and transitional care services to medically complex and recently discharged patients throughout the San Fernando Valley and Antelope Valley.

This field-based position offers clinical autonomy while working alongside an interdisciplinary care team focused on improving patient outcomes, reducing hospital readmissions, and supporting patients as they recover safely in the comfort of their homes.

Compensation & Benefits:

• Base salary $119,313 – $178,968 annual

Qualifications:

• Master of Science in Nursing
• Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted California RN license
• Active, unrestricted California NP license
• Minimum of 1 year of Nurse Practitioner clinical experience
• Strong assessment, diagnostic, and care management skills
• Valid driver's license and reliable transportation
• Ability to work independently while collaborating with a multidisciplinary team
• Current TB test or willingness to obtain prior to employment
