Adult Site Physician

We are seeking a Board-Certified Site Physician to join our team in Brooklyn, New York and provide comprehensive medical care to patients at a Community Health/Day Treatment Center. This position offers a consistent Monday–Friday schedule and the opportunity to make a meaningful impact caring for a frail and elderly patient population. The Site Physician will oversee the professional, clinical, organizational, and administrative aspects of patient care and work closely with an interdisciplinary team to coordinate high-quality, patient-centered services.

Compensation & Benefits

• Base salary $240,000–$280,000 depending on experience, plus bonus 
• Comprehensive benefits package
• Full-time Monday–Friday schedule, 8:00 AM–5:00 PM
• Combination of site-based and home-care services
• Collaborative interdisciplinary practice environment

Requirements:

• Graduate of an accredited medical school.
• Completion of Internal Medicine or Family Medicine residency.
• Board Certified in Internal Medicine or Family Medicine required.
• Geriatric Medicine certification, fellowship, or experience preferred.
• Minimum one year of experience caring for frail or elderly patients preferred.
• Home-care experience preferred; must be willing to provide home-care services.
• Current, unrestricted New York State medical license.
• Active DEA license.
• Current CAQH profile.
• Active Medicare and Medicaid provider IDs.
• Excellent clinical, communication, organizational, and interpersonal skills.
• No malpractice or loss-of-license history related to practice at the DTC.
• Bilingual skills a plus
