Clinical Lead Adult Nurse Practitioner

We are seeking an experienced Clinical Lead Nurse Practitioner to join our TeamCare program in Bronx, New York. The Clinical Lead NP will be assigned to a TeamCare site and provide comprehensive care to adult patients while serving as a clinical leader within the practice. This position combines direct patient care with clinical leadership and care coordination. In the absence of, or at the direction of, the Medical Director, the Clinical Lead NP will represent the Medical Director and provide clinical leadership at the assigned location.

Schedule: Monday-Friday 8:30-5:30 (1 hour lunch)

Compensation & Benefits

• Base salary $162,000–$190,000 depending on experience, plus bonus

Requirements

• Graduate of an accredited Nurse Practitioner program and eligible for NYSED recognition.
• 2+ years of clinical experience in primary care, home care, care management, or related settings.
• 1+ year of experience caring for adult patients preferred.
• Current, unrestricted NY Nurse Practitioner and RN licenses.
• Current BLS certification.
• Active CAQH profile.
• Current Medicare and Medicaid provider IDs.
• AANP or ANCC Board Certification and current DEA registration.
• Strong clinical judgment, communication, analytical, and organizational skills.
• Proficiency with Microsoft Office and electronic clinical systems.
• Bilingual skills a plus
