Nurse Practitioner

Looking for a high-paying NP job with a 4-day workweek and a schedule you control?

You’d be the physiatry consult provider for three skilled nursing facilities near Rockford. On site 3 to 4 hours a day seeing 20 to 25 focused rehab consults, then chart from home with an AI scribe. Mon-Thu or Tue-Fri.

4 days/week, no nights, no weekends, no call

Schedule

• You set your own rotation. Each building twice a week, never back-to-back days in the same building.
• Two heavy days and two light days or spread it out. Your call.
• Documentation due within 48 hours, done from home.
• A remote collaborating physiatrist backs every case.

The work
Rehab consults only: function, mobility, pain, therapy planning, discharge. Not primary care.

Pay

• Base $120K to $150K plus RVU production bonus. W-2 with benefits.
• Group is in 300+ facilities in 18 states and growing.

Requirements

• NP with an active Illinois license.
• A few years as a provider. Hospice, home health, primary care, SNF, or rehab backgrounds all fit. New grads with LTC experience considered.
